Job summary

Sienna Senior Living is seeking an RPN RAI Coordinator at Weston Terrace in Toronto. You will lead the planning, coordination, and evaluation of the RAI program, ensuring adherence to MOHLTC guidelines and organizational policies.

You will work in a collaborative team environment to optimize resident assessments and outcomes. The role requires current RPN status in Ontario, RAI certification (or willingness to obtain), and strong analytical and problem‑solving skills.

Qualifications

  • Must be a Registered Practical Nurse in good standing with the Ontario College of Nurses.
  • RAI certification as per MOHLTC requirements is expected or asset.

Responsibilities

  • Plan, implement, coordinate, and evaluate the RAI Program to ensure compliance with standards and MOHLTC requirements.
  • Establish and communicate resident assessment schedules to the inter-professional team.
  • Ensure RAI accuracy and update coding, care plans, and reviews for residents.

What you’ll be doing:

The RPN RAI Coordinator is responsible for the planning, implementation, coordination, and evaluation of the RAI Program, ensuring compliance with quality management standards, legislative requirements, and organizational policies and procedures.

You will be a welcomed member team and impact in the following ways:

  • Establish, maintain, and communicate current resident assessment schedule to inter-professional team.
  • Ensure RAI accuracy and compliance with coding, capping and care plans are updated.
  • Reviewandanalyzeeachresident’sRAIassessmentstoensureaccuracyandmeet theresidentsneeds.
  • Monitor and analyzes RAI outputs and reports.
  • Produce RAI reports for leadership use to assist in clinical and operational decision-making
  • Responsible for managing the computerized data submission process and maintain accurate records in accordance with Ministry of Health and Long-Term Care (MOHLTC).Collaborate with the leadership team and support services consultants to ensure data integrity and effective data utilization.
  • Monitors and analyzes RAI outputs and reports, and communicates findings with the leadership team and support services committees.
  • Lead the RAI education process for all participating team members
Who You Are:

A Registered Practical Nurse in good standing with the Ontario College of Nurses

  • Successful annual completion of RAI certification as per Ministry of Health requirements for Relias-MLTC
  • RAI experience/education is an asset
  • Good problem solving and analytical skills
