Generations Toronto is seeking a dedicated RAI Coordinator (RPN or RN) to lead MDS implementation for the interdisciplinary team at a long‑term care setting. You will coordinate assessments, mentor staff, and ensure timely documentation in compliance with LTC standards.
The role requires a strong background in Long Term Care, Ontario regulatory knowledge, and at least three years of clinical or managerial experience. Position is full-time on-site with AgeCare as manager of the home.

The RAI (Resident Assessment Instrument) Coordinator is responsible for the overall administration, organization, education, co‑ordination, and implementation of the Minimum Data Set (MDS) for the interdisciplinary team. Provides supervision and assistance to the interdisciplinary team regarding MDS documentation.
