Job summary

The RPN - RAI Coordinator position at Muskoka Shores Community in Gravenhurst, part of Sienna Senior Living, coordinates the RAI program under the Director of Care. Plan, implement and monitor resident assessments to ensure accuracy and MOHLTC compliance while supporting inter-professional teams.

Qualifications

  • Registered Practical Nurse in good standing with the Ontario College of Nurses.
  • RAI certification as per MOHLTC requirements.
  • RAI experience/education is an asset.
  • Good problem solving and analytical skills.

Responsibilities

  • Plan, implement, coordinate and evaluate the RAI program.
  • Maintain resident assessment schedule with inter-professional team.
  • Ensure RAI accuracy and update care plans.
  • Review and analyze each resident's RAI assessments for accuracy.
  • Monitor RAI outputs and produce leadership reports.
  • Manage computerized data submission per MOHLTC; ensure data integrity.
  • Lead RAI education for team members.

What you’ll be doing:

TheRPNRAI Coordinatoris responsible for the planning, implementation, coordination, and evaluation of the RAI Program, ensuring compliance with quality management standards, legislative requirements, and organizational policies and procedures.

You will be a welcomed member team and impact in the following ways:

  • Establish, maintain, and communicate current resident assessment schedule to inter-professional team.
  • Ensure RAI accuracy and compliance with coding, capping and care plans are updated.
  • Reviewandanalyzeeachresident’sRAIassessmentstoensureaccuracyandmeet theresidentsneeds.
  • Monitor and analyzes RAI outputs and reports.
  • Produce RAI reports for leadership use to assist in clinical and operational decision-making
  • Responsible for managing the computerized data submission process and maintain accurate records in accordance with Ministry of Health and Long-Term Care (MOHLTC).Collaborate with the leadership team and support services consultants to ensure data integrity and effective data utilization.
  • Monitors and analyzes RAI outputs and reports, and communicates findings with the leadership team and support services committees.
  • Lead the RAI education process for all participating team members
Who You Are:

ARegisteredPracticalNurseingoodstandingtheOntario CollegeofNurses

  • Successful annual completion of RAI certification as per Ministry of Health requirements for Relias-MLTC
  • RAI experience/education is an asset
  • Good problem solving and analytical skills
