Job summary

Extendicare is seeking an experienced MDS Coordinator to oversee MDS assessments across our long-term care homes in Canada. You will coordinate quarterly submissions, analyze data, and lead staff training on RAI content and RUG classification.

You will collaborate with Health Zone, AH&W, CIHI, and PCC to ensure accuracy and improve care outcomes while contributing to program development and quality improvement initiatives.

Qualifications

• 3-5 years in Long Term Care with MDS experience is asset.
• Strong analytical and assessment skills.
• Ability to lead training and education with adult learners.

Responsibilities

• Coordinates completion of MDS assessment on all residents within timelines.
• Leads integration of MDS with clinical tools and streamlines processes.
• Analyzes data at multiple aggregation levels for decision support.
• Communicates RAP triggers and care plans benefits to staff.
• Acts as liaison on RAI data with Health Zone/AH&W/CIHI/PCC as appropriate.
• Develops facility materials for PCC program and manages RAI access.
