Job summary

AgeCare's Registered Practical Nurse (RPN) provides professional nursing care to residents per required medical care and nursing standards. The RPN also directs Personal Support Workers under College of Nurses standards.

In collaboration with the RN, you will ensure care plans are followed, liaise with physicians, and participate in shift reports to keep the team informed of resident status changes. Ontario licensure is required.

Qualifications

• Geriatric/rehabilitation/long-term care nursing background, including activation and personnel administration.
• Knowledge of applicable acts and College of Nurses standards relevant to long-term care.
• Current Certificate of Competence from the College of Nurses of Ontario and active membership.

Responsibilities

• Collaborate with the RN to implement AgeCare policies and procedures on the unit.
• Liaise with physicians and health care team; notify physician of significant changes when RN is absent.
• Work with the health care team to meet residents' care needs.
• Direct Personal Support Workers on the care team.
• Participate in shift reports to inform the team of changes in resident status.
