Job summary

Extendicare is seeking a part-time Registered Practical Nurse to provide planned, patient-centered nursing care in our long-term care setting in Ottawa, Ontario. You will work within an interprofessional team, manage care plans, and administer medications in line with provincial standards and Extendicare policies.

You will collaborate with DOC/DHW and other healthcare professionals to coordinate admissions, transfers and care conferences.

Qualifications

• Current registration as a Registered Practical Nurse in applicable Province/State.
• Experience working in a senior's environment and with unregulated care providers preferred.
• Education and experience in gerontology, rehabilitation, community/long term care an asset.
• Strong written and oral communication, critical thinking, care coordination, time management and customer focus skills.
• Current Basic Cardiac Life Support and First Aid certification required.
• Computer proficiency and experience with Microsoft office applications.
• Current (within 6 months) Vulnerable Position/Sector Screening (VPS).

Responsibilities

• Provide planned, patient-oriented nursing care in accordance with provincial standards.
• Complete and ensure resident care assessments, care/service plans and documentation are accurate and up-to-date.
• Implement and evaluate care/service plans and communicate changes as required.
• Assist with scheduling resident care conferences and participate where applicable.
• Work with DOC/DHW to facilitate moves, transfers and admissions/discharges.
• Administer medications and treatments in compliance with regulations and Extendicare policies.
• Coordinate care with physicians, nursing and interdisciplinary teams and contracted providers.
• Coordinate nursing team assignments for efficient and timely service delivery.
• Respond to significant changes in resident condition and medical emergencies.
• Educate residents and families; delegate to unregulated care providers as appropriate.
• Support infection prevention and control and health and safety initiatives in the nursing department.
