Job summary

Extendicare is hiring a part-time Registered Practical Nurse (RPN) to provide planned, patient-centered nursing care in compliance with provincial standards. You will work with an interprofessional team to assess, plan, implement, and evaluate resident care while documenting changes and communicating with the care team.

The role requires current RPN registration in Canada, LTC experience, BCLS, First Aid, and MS Office proficiency.

Qualifications

• Current registration as a Registered Practical Nurse (RPN) in Canada/Province.
• Experience in senior care and with unregulated care providers preferred.
• Strong communication, critical thinking, time management and care coordination.
• Current Basic Cardiac Life Support and First Aid certification.
• Computer proficiency with MS Office.

Responsibilities

• Provide planned, patient‑centered nursing care per provincial standards.
• Maintain accurate resident assessments, care/service plans and documentation.
• Implement and evaluate care plans and communicate changes.
• Assist with scheduling care conferences and participate where applicable.
• Collaborate with DOC/DHW to coordinate moves and transfers.
• Administer medications and treatments following regulations and Extendicare policies.
• Provide resident/family education.
• Coordinate team assignments for timely care delivery.
• Respond to changes in condition and emergencies; provide first aid.
