Part-Time RPN: Senior Care & Care Coordination

Job summary

Extendicare is hiring a Registered Practical Nurse (RPN) for part-time shifts in our Ontario long-term care locations. You will deliver planned, patient-centered nursing care under the supervision of the DOC/DHW and work with an interdisciplinary team to support residents.

Responsibilities include assessments, care planning, medication administration, and education for residents and families. A valid RPN license in Ontario, BCLS, First Aid, and VPS clearance are required.

Qualifications

• Must be registered as a Registered Practical Nurse in the applicable Province/State.
• Experience in a senior’s environment and with unregulated care providers preferred.
• Education and experience in gerontology, rehabilitation, community/complex/long term care an asset.
• Strong written and oral communication, critical thinking, care coordination, time management and customer focus skills.
• Current Basic Cardiac Life Support and First Aid certification required.
• Computer proficiency and experience with Microsoft Office applications.
• Current (within 6 months) VPS (Vulnerable Position/Sector Screening).

Responsibilities

• Work as a member of the inter professional team following Extendicare’s Practice Model and person-centered care approach.
• Complete and ensure resident care assessments, care/service plans, and documentation are accurate and up to date.
• Implement, evaluate and adjust care/service plans as needed.
• Assist with scheduling of resident care conferences and participate where applicable.
• Coordinate nursing team assignments to ensure timely delivery of services and respond to resident condition changes or emergencies.
