Job summary

Extendicare Canada seeks a Registered Practical Nurse (RPN) to deliver planned, patient-centered nursing care in line with provincial standards. You will report to the Director of Care or Director of Health and Wellness and collaborate with an interprofessional team to implement and evaluate care plans.

Qualifications

• Must have strong written and oral communication, critical thinking, care coordination, and time management skills.
• Current registration as a Registered Practical Nurse in the applicable Province/State.
• Experience in geriatrics, rehabilitation, or long-term care is an asset.

Responsibilities

• Provide planned, patient-oriented nursing care per provincial standards.
• Complete and document care plans and assessments; update as required.
• Administer medications and treatments and ensure regulatory compliance.
• Coordinate with DOC/DHW and interprofessional team for care conferences and transfers.
• Educate residents and families on care plans and health conditions.
• Lead unregulated care providers and monitor infection control and safety practices.
