Job summary

Extendicare is seeking an experienced Registered Practical Nurse (RPN) in Ontario to provide planned, patient-centered nursing care under the supervision of the DOC/DHW. You will work with an interprofessional team to implement, monitor, and evaluate care plans, while ensuring accurate documentation and regulatory compliance.

The role emphasizes resident safety, timely care delivery, and collaboration with nursing staff to manage moves, conferences, and emergencies.

Qualifications

• Current registration as a Registered Practical Nurse in Ontario.
• Experience in geriatric or long-term care is preferred.
• Strong written and oral communication and critical thinking.
• Ability to coordinate care plans and work with interdisciplinary team.
• Must have recent CPR/First Aid certification and VPS within 6 months.

Responsibilities

• Provide direct resident care and administer medications/treatments.
• Develop, implement, and evaluate resident care plans.
• Coordinate scheduling and participate in care conferences.
• Document care accurately and communicate changes to the care team.
• Collaborate with DOC/DHW and interdisciplinary team for transfers and admissions.
