Job summary

NSPHA seeks a Tenant Relations Coordinator to provide social work services to public housing tenants in the Western district. The role involves collaborating with residents, identifying needs, and promoting health and safety to maximize quality of life.

The incumbent will conduct assessments, develop case plans, refer to community resources, and work with families, seniors, and support networks to ensure tenancy stability.

Qualifications

• Bachelor's degree in a related field plus five years' experience.
• Extensive knowledge of health and social service systems, including long term care and placement procedures.
• Ability to balance clients' needs with available resources.
• Ability to adapt to and work with a variety of situations and people.
• Working knowledge of desktop applications (Microsoft Office).
• Ability to relate well to all kinds of people and build rapport with stakeholders.
• Interact effectively with clients, service coordinators, staff and community groups.

Responsibilities

• Provide social services to tenants to enhance safety and independence.
• Collaborate with NSPHA staff to respond to referrals involving complex issues.
• Conduct home visits to assess risk and determine service needs.
• Refer clients to community resources and develop appropriate referrals.
• Develop a case plan and address risk and need for clients.
• Provide follow-up and reassess services for effectiveness.
• Coordinate with community agencies and programs like Continuing Care Nova Scotia.
• Maintain client records and communicate with coordinators for a unified service.
• Participate in case conferences and liaison meetings.
• Engage in community development initiatives for seniors and families.
