Responsibilities

• Work as part of an inter-professional team applying person-centered care.
• Complete and maintain resident clinical care assessments, care/service plans and documentation.
• Implement, evaluate and adjust care/service plans as needed.
• Assist with scheduling and participate in resident care conferences.
• Support moves, transfers and coordination with the Director of Health & Wellness.
• Provide direct resident care and administer medications/treatments per regulatory standards.
• Educate residents and families, coordinate with physicians and nursing/interdisciplinary teams.
