Job summary

McKesson is seeking a compassionate and patient-focused Part-Time Nurse Case Manager to support patients, healthcare providers, and clinic partners through a specialized Patient Support Program (PSP). This role involves education, care coordination, and ongoing support to help patients navigate treatment and reimbursement, collaborating with physicians, clinics, pharmacies, and internal stakeholders.

It is a part-time position requiring approximately 2.5 days per week, with hours 8:00 a.m.

Qualifications

• Active and unrestricted nursing license in Alberta and/or British Columbia (RN, RPN/LPN, or equivalent).
• Diploma, degree, or equivalent combination of education and experience consistent with the P1 Professional Career Framework.
• Strong verbal and written communication skills.
• Proficiency using Microsoft Office and electronic documentation systems.
• Ability to travel to clinics and healthcare sites throughout Alberta and British Columbia.
• Ability to work approximately 2.5 days per week within program operating hours (8:00 a.m. to 8:00 p.m. EST).
• Excellent organizational skills and attention to detail.

Responsibilities

• Serve as primary contact for patients in the Patient Support Program.
• Educate patients on prescribed therapies, expectations, administration, and adherence.
• Support patient enrollment and onboarding activities.
• Coordinate care with physicians, clinics, pharmacies, and other healthcare professionals.
• Assist patients in navigating access to treatment and reimbursement processes.
• Conduct patient assessments and follow-ups as required by the program.
• Monitor patient progress and provide ongoing support throughout the treatment journey.
• Maintain accurate documentation within program systems.
• Identify and elevate clinical or operational concerns as appropriate.
• Contribute to quality improvement initiatives and program success measures.
• Build and maintain positive working relationships with healthcare providers and clinic partners.
