Job summary

United States Digital Space LLC in Canada is seeking a compassionate Part-Time Nurse Case Manager to support patients, healthcare providers, and clinic partners through a specialized Patient Support Program (PSP).

As a key member of the care team, you will educate, coordinate care, and provide ongoing support. You will collaborate with physicians, clinics, pharmacies, and internal stakeholders to facilitate timely access to treatment and deliver an exceptional patient experience.

Qualifications

• Active and unrestricted nursing license in Alberta and/or British Columbia.
• Diploma, degree, or equivalent combination of education and experience.
• Strong verbal and written communication skills.
• Proficiency using Microsoft Office and electronic documentation systems.
• Ability to work approximately 2.5 days per week within 8:00 a.m. to 8:00 p.m. EST, Mon–Fri.
• Ability to travel to clinics across Alberta and British Columbia.
• Excellent organizational skills and attention to detail.

Responsibilities

• Serve as primary point of contact for patients enrolled in the PSP.
• Provide education regarding therapies, treatment expectations, medication administration, and adherence strategies.
• Support patient enrollment and onboarding activities.
• Coordinate care with physicians, clinic staff, pharmacies, and healthcare professionals.
• Assist patients in navigating access to treatment and reimbursement processes.
• Conduct patient assessments and follow-ups as required.
