Job summary

McKesson is seeking a compassionate Part-Time Nurse Case Manager to support patients in a specialized Patient Support Program across Alberta and British Columbia. This remote role focuses on education, care coordination, and helping patients navigate access to treatment while collaborating with clinicians and pharmacies. The role requires about 2.5 days per week within 8:00 a.m.

to 8:00 p.m. EST hours, with travel to clinics as needed.

Qualifications

• Active and unrestricted nursing license in Alberta and/or British Columbia.
• Diploma or degree in nursing or equivalent.
• Strong verbal and written communication skills.
• Ability to travel to clinics across Alberta and British Columbia.

Responsibilities

• Serve as primary point of contact for patients in the PSP.
• Provide education on therapies, treatment expectations, and adherence.
• Support patient enrollment and onboarding.
• Coordinate care with physicians, clinics, pharmacies and healthcare teams.
• Assist patients with access to treatment and reimbursement processes.
• Conduct patient assessments and follow-ups.
• Maintain documentation in program systems.
• Escalate clinical or operational concerns as needed.
• Contribute to quality improvement initiatives.
