Job summary

United States Digital Space LLC seeks a compassionate Part-Time Nurse Case Manager to support patients, healthcare providers and clinic partners via a specialized Patient Support Program (PSP). You will educate patients, coordinate care, and help ensure timely access to treatment with an exceptional patient experience.

This remote role is ~2.5 days/week, requiring Alberta/BC licensure, strong communication, and travel to clinics across Alberta and British Columbia.

Qualifications

• Active nursing license in Alberta and/or British Columbia (RN/RPN/LPN).
• Diploma, degree, or equivalent nursing education.
• Strong verbal and written communication skills.
• Proficiency using Microsoft Office and electronic documentation systems.

Responsibilities

• Serve as a primary point of contact for patients enrolled in the PSP.
• Educate patients on prescribed therapies, treatment expectations, and adherence.
• Support patient enrollment and onboarding activities.
• Coordinate care and communicate with physicians, clinics, and pharmacies.
• Assist patients in navigating access to treatment and reimbursement processes.
• Conduct patient assessments and follow-up activities as required.
