Job summary

Wesley in Halton is seeking an Intensive Case Manager to support intake and day-to-day administration for housing services. The role focuses on housing first, harm-reduction approaches and coordinated supports for homeless individuals and families.

The position is unionized, full-time and temporary through July 1, 2026, with a pay rate of $24.38 per hour. Reporting to the Manager of Halton Housing Services, the role requires flexibility and strong collaboration with partners.

Qualifications

• Post-secondary degree/diploma in Human Services or related field.
• Registration and member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required.
• Experience with adults and families with mental health and addictions issues and homelessness.
• Must be able to work flexible shifts, including day, evening, and weekend hours as well as 24-hour intake responsibilities.
• Must be able to meet travel expectations of the position; a valid Driver’s license and access to a vehicle is an asset.

Responsibilities

• Works with assigned families and individuals in a housing program towards a housing plan.
• Responds to calls and referrals from community agencies per caseload.
• Places individuals and families into permanent leased units.
• Assesses needs for services to support transition to stable housing.
• Completes Social & Community Housing applications (HATCH) to Region for approval.
• Assess clients for domestic violence and make referrals.
• Assist participants with moves into independent housing and orientation.
• Maintain communication with housing providers, social partners, and landlords.
• Report to CAS if child abuse/neglect is suspected.
• Conduct visits to provide life skills, budgeting, and housing-related supports.
• Maintain accurate record-keeping as per Wesley.
• Supports approximately 1 shift per month of 24-hour intake; flex time available.
• Other duties as assigned.
