Job summary

SOAR Community Services in Brantford, ON is seeking a Temporary Full Time Housing Resource Centre Case Manager to assist individuals and families experiencing or at risk of homelessness. You will support housing applications, provide resource referrals, and help clients learn tenancy rights.

You’ll work with the Housing Services team, apply a harm-reduction approach, and ensure accurate documentation across databases for reporting and program integrity.

Qualifications

• Minimum 2 years’ combined student placement and/or paid experience with individuals facing homelessness or with substance use/mental health needs.
• Proficient in MS Office and familiar with databases.
• Valid Driver’s License and ability to visit client homes or public locations.

Responsibilities

• Deliver direct client service to individuals and families experiencing homelessness.
• Complete SPDAT assessments and support housing applications and benefits.
• Collaborate with Housing Services to ensure program delivery.
• Apply a harm-reduction approach in service delivery.
• Maintain documentation in multiple electronic databases for reporting.
• Participate in department and sector case management meetings.
• Liaise with landlords and referring agencies as needed.
