Job summary

WoodGreen Community Services is recruiting a Case Manager for the Street to Home Housing Program in Toronto. This role supports tenants in supportive housing with complex needs, focusing on hoarding/pest control interventions and coordinated care.

You will work within a multidisciplinary team to assess, plan, implement, and monitor client progress, linking clients to health services and community resources while maintaining comprehensive documentation and adherence to policies.

Qualifications

• Bachelor's degree in Social Work or related discipline.
• 3+ years experience in mental health, harm reduction, and housing support.
• Ability to work within a multidisciplinary team and collaborate with other professionals.

Responsibilities

• Provide comprehensive case management including assessment, planning, and crisis intervention.
• Develop case plans with individuals and coordinate services.
• Assist tenants with mental health support, life skills, and resources.
• Coordinate referrals to primary care and community resources.
• Maintain records, reports, and ensure program compliance.
