Job summary

CMHA Peel Dufferin in Brampton is hiring a Transitional Case Manager for the Brampton HART Hub to provide case management within a Housing First framework. You will conduct psychosocial assessments, develop care plans, link clients to community resources, and support transitions from homelessness into housing and recovery.

The role is full-time, based onsite at the Brampton HART Hub, with a multidisciplinary team and opportunities for professional development.

Qualifications

• Educational background as noted in requirements.
• Experience supporting mental health, addiction, and homelessness challenges is required.
• Knowledge of diverse populations and social determinants of health is essential.

Responsibilities

• Conduct comprehensive psychosocial assessments.
• Provide case management and activities of daily living support.
• Assess risk and use crisis intervention and de-escalation strategies.
• Refer clients to community resources and groups for recovery goals.
• Facilitate recovery and social-recreational groups as needed.
• Participate in meetings, trainings, and events.
• Document all client contact within 24 hours.
