Job summary

Placements24 is seeking an experienced Remote Disability Support Coordinator to join our team. You will work with clients, families, and service providers to develop personalized support plans and coordinate disability services entirely remotely.

The ideal candidate has a bachelor’s degree in a related field and at least 3 years of experience in disability services, with strong knowledge of legislation, excellent communication, and solid organizational skills.

Qualifications

Responsibilities

  • Assess client needs and develop comprehensive, person-centered support plans.

  • Liaise with healthcare professionals, government agencies, and community organizations for integrated support.
  • Monitor effectiveness of plans and adjust as needed to ensure client satisfaction.

  • Maintain accurate client records in a secure digital environment.

About the Role

Key Responsibilities
