Job summary

Bayshore HealthCare in Montreal is seeking a Case Manager to coordinate patient journeys from welcome calls through discontinuation of therapy. You will work with physicians, DANs, nurses and patients, ensuring timely access to treatment and ongoing support in a fast-paced, team-driven environment.

The role requires bilingual English/French communication, strong customer service, and solid data-entry skills. This is a contract position with a November 2027 anticipated end date.

Qualifications

• Demonstrated ability to deliver exceptional customer service and foster positive relationships with stakeholders.
• Bilingual communication skills in English and French.
• Strong problem-solving and interpersonal abilities.
• Accurate data entry and documentation to support reporting requirements.

Responsibilities

• Ensure patient enrollment in the PSP and provide a warm welcome to the program.
• Provide direct telephone assistance to patients needing reimbursement support.
• Develop strong relationships with patients, caregivers and healthcare teams.
• Navigate reimbursement options to maximize coverage via private and public payers.
• Represent patients with insurers for Special Authorization and Prior Authorization forms when applicable.
• Coordinate with pharmacy for medication coverage and dispensing; troubleshoot billing issues.
• Maintain ongoing communication with the healthcare team and ensure accurate records.
• Identify gaps and opportunities to improve the patient experience.
• Adhere to Bayshore policies and participate in quality initiatives.
