Job summary

Sentrex Health Solutions Inc. is seeking a Bilingual Nurse Case Manager (NCM) to coordinate and deliver patient support programs. You will liaise with healthcare providers, patients, and insurers to secure coverage and ensure timely access to therapies.

Requirements include RN/RPN licensure and 2+ years of clinical experience, with bilingualism in English and Mandarin. The role offers a remote, Canada-wide opportunity with competitive benefits and ongoing professional development.

Qualifications

• Must be an RN or RPN in good standing with the province
• Experience in Patient Support Programs is required
• Reimbursement knowledge to obtain coverage for patients is an asset
• Experience in Hepatitis is a strong asset
• Minimum 2 years of clinical experience (preferred)
• Bilingualism (English and Mandarin) is required
• Bilingual French is an asset

Responsibilities

• Register HCP and patient according to program protocols
• Data entry of program service information with accuracy
• Answer general inquiries about program services and diseases
• Provide medical education to patients and HCPs
• Attend training sessions for specific therapies
• Provide telephone support (inbound/outbound)
• Adhere to case-management protocols and manage patient follow-up
• Assist with funding/coverage opportunities for therapies
• Attend patient meetings to educate patients and providers
• Identify and report Adverse Events to manufacturers and teams
• Maintain confidentiality of patient and corporate information
• Coordinate with physicians, pharmacies and labs to deliver services
• Pursue ongoing continuing education and training materials
• Enrol patients and follow-up as per protocols
• Advocate for patients and assist with coverage renewals
