Job summary

Sentrex Health Solutions Inc. is seeking a Care Navigator to manage enrollment, reimbursement processes, and service coordination for patients in the PSP.

You will assist physicians and patients with drug coverage, process submissions, and ensure timely access to prescribed therapies while delivering continuous patient support. The role requires strong communication, knowledge of private and public reimbursement structures, and the ability to work from a home office.

Qualifications

• Post-secondary education with healthcare experience is an asset.
• Bilingualism in English and French is an asset; in Quebec it is required.
• Experience with reimbursement billing and public/private reimbursement structures is an asset.
• Must be able to work from home with a quiet home office space.
• Excellent verbal and written communication skills.
• Strong analytical skills including interpretation of regulation and legislation.

Responsibilities

• Act as the primary point of contact for all patient elements of the PSP.
• Responsible for timely enrollment of patients into the PSP.
• Provide telephone support including maintenance and adherence calls.
• Collaborate with patient, insurer, and physician on documentation for reimbursement.
• Review patient status and assist physicians with documentation for coverage.
• Provide therapy guidelines/education to manage expectations.
• Collect information and conduct patient financial eligibility assessments.
• Coordinate with physicians' offices, pharmacies, and service providers.
• Ensure services are coordinated and tracked for quick access to therapy.
• Review and validate information to ensure initiation and continuation of services.
• Maintain service levels in case management and KPIs per SOPs.
• Maintain CRM data accuracy and timeliness.
• Foster teamwork and report obstacles to PSP Supervisor.
• Follow up until coverage decisions are obtained.
• Maintain confidentiality of patient, client, and corporate information.
