Job summary

Qualifications

  • Registered Nurse (RN) licensing with CNO in good standing.

  • On-call coverage including after hours to ensure patient safety.

Job description

In this community RPN Visiting Nursing role, as a Registered Practical Nurse (RPN) you will have scheduled visits to care for patients in various settings. You will work with patients of all ages with differing complexity of palliative care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.

What you’ll do:
  • Using appropriate assessment tools/techniques to assess patients’ individualized needs
  • Executing and continuously monitor care plans
  • Engaging patients using a collaborative approach by building rapport and trust
  • Maintaining and update professional patient records related to patient encounters and plans

  • Registration with the CNO as a Registered Nurse in good standing
