Job summary

ParaMed is seeking a Registered Nurse for visiting nursing across the Burlington, Halton region. You will provide in‑home care, perform assessments, implement care plans, and support patients to stay in their homes comfortably.

Qualifications

• Registered Nurse with CNO RN registration in good standing.

Job Description In this community RN Visiting Nursing role, as a Registered Practical Nurse (RPN) you will have scheduled visits to care for patients in various settings. You will work with patients of all ages with differing complexity of palliative care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.

• Using appropriate assessment tools/techniques to assess patients’ individualized needs
• Executing and continuously monitor care plans
• Engaging patients using a collaborative approach by building rapport and trust
• Maintaining and update professional patient records related to patient encounters and plans

• Registration with the CNO as a Registered Nurse in good standing
• Valid driver’s license and access to a vehicle is required

• Willingness to travel to and from patient residences within a designated area
