Job summary

ParaMed is seeking a Clinical Planner to onboard new patients requiring Personal Support Services in an assigned district. You will assess needs, develop and implement patient care plans, and ensure safe work environments for staff.

The role requires RN/LPN registration with a provincial body, strong clinical knowledge, and solid organizational and communication skills. Travel to patient sites may be required as part of the role.

Qualifications

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing with the provincial body.
  • Knowledge of Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / PHI Act and OHS standards.
  • Experience with community health care and services best practices.
  • Strong oral and written communication skills.
  • Excellent organizational skills and attention to detail.
  • Knowledge of community resources for patients and families.
  • Demonstrated patient-centered care.
  • Ability to obtain VPS criminal background check and drive a personal vehicle if required.

Responsibilities

  • Perform the initial assessment of care needs for new patients and establish individual care plans for patient services.
  • Complete the initial safety assessment of a new patient's living environment to ensure safe working conditions for staff.
  • Perform the initial personal care services for new patients.
  • Advise Operations on scheduling parameters for ongoing patient visits based on care requirements.
  • Inform patients and families of ParaMed policies, scheduling practices, and expected time of care.
  • Maintain current and accurate patient records.
  • Respect cultural needs, dignity, privacy, and confidentiality of patients and staff.
  • Stay updated on changes in practice that affect scope of practice.

The Clinical Planner is accountable for onboarding new patients requiring Personal Support Services within an assigned district, through the assessment, implementation, and evaluation of a wide range of clinical and personal care interventions.

What you’ll do
  • Perform the initial assessment of care needs for new patients and establishes the individual care plans for patient services.
  • Complete the initial safety assessment of a new patients’ living environment to ensure employees will be working in safe and healthy work conditions.
  • Perform the initial personal care services for new patients.
  • Advise Operations of the appropriate parameters for scheduling the patient’s ongoing visits, based on clinically indicated service requirements.
  • Inform patients and families of ParaMed policies and scheduling practices, as well as the expected time of care.
  • Maintain current and accurate patient records.
  • Show respect for cultural needs, dignity, privacy, and confidentiality of patients and staff.
  • Stay abreast of changes in practice setting that affect scope of practice.
What you bring
  • Registration in good standing as a RPN/LPN with provincial governing bodies and minimum two years’ experience as a practicing nurse.
  • Working knowledge of applicable legislation including but not limited to Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / Personal Health Information Act, Occupational Health and Safety legislation and practices.
  • Knowledgeable in best practice standards for community health care and services.
  • Strong oral and written communication skills
  • Excellent organizational skills
  • Demonstrated competency in patient centered care principles and practices.
  • Knowledgeable of community resources available for patients and families.
  • Strong independent critical thinking and problem-solving skills.
  • Ability to provide a current, clear criminal back check with Vulnerable Position Screening (VPS)
  • Access to a personal vehicle required
