Job summary

SE Health is seeking a registered nurse or registered practical nurse in Fergus, Ontario. You will assess clients, develop and evaluate care plans, and lead care for your caseload in collaboration with a multidisciplinary team.

The role requires current Ontario certification, full‑time availability with alternating weekends, and a commitment to full scope nursing. A CPR certificate and immunizations are required; travel to local areas is possible.

Qualifications

• Current Certificate of Competence with the College of Nurses of Ontario as RN/RPN.
• Availability to work FT days with every other weekend and on-call component (Evening and Overnight 6 nights/month).
• Current CPR certificate and immunization records up to date.

Responsibilities

• Complete comprehensive client assessments and develop care plans.
• Lead client care as primary provider for a caseload, collaborating with the care team.
• Support clients through transitions by connecting with community services.

Responsibilities:

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs


• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.


• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements:

• Current Certificate of Competence with the College of Nurses of Ontario as RPN.


• Availability to work FT days with every other weekend and on call component (Evening and Overnight 6 nights/month.)


• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.


• A willingness to attain required SE Health certifications


• A current CPR certificate is required.


• This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.


• Required to travel to Fergus/Elora/Drayton/Arthur and surrounding areas.
