Job summary

SE Health in Ontario seeks a licensed RN/RPN to provide community-based care. You will assess clients, plan and coordinate care with the team, and support transitions at home.

The role offers flexible shifts, paid training and opportunities to grow. Must hold Ontario nursing registration, CPR, a valid driver’s license, up-to-date immunizations, and willingness to obtain Saint Elizabeth certifications; travel is required.

Qualifications

• Current registration with the College of Nurses of Ontario.
• CPR certificate required; immunization records up to date.
• Valid driver’s license and vehicle; travel required.
• Willingness to attain Saint Elizabeth certifications.
• Availability for a mix of days, evenings, overnights, weekends and on-call shifts.
• Physical ability to lift up to 55 pounds; mobility required.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care per best practices and client needs.
• When designated as primary provider, act as point of care lead for the client caseload.
• Support client through care transitions by coordinating with community services.

Responsibilities:

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements:

• Current registration with the College of Nurses of Ontario
• Canadian Nurses Association (CNA) specialty certification an asset
• Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care
• A current CPR certificate is required
• A valid driver’s license and vehicle (travel required)
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required Saint Elizabeth certifications.
• Availability to work a mix of days, evening and overnight, weekends and on call shifts.
• This is a physically demanding position and requires hand, arm and shoulder dexterity, sensory ability, lifting to a maximum of approximately fifty-five (55) pounds, walking, climbing stairs, standing, bending, reaching, and frequent movement
