Job summary

SE Health in Fergus, Ontario is seeking a Registered Practical Nurse to provide comprehensive home care in clients' communities. You will assess, plan, implement and evaluate care with a patient-centered approach and collaborate with the care team to deliver high-quality, safe services.

The role requires current Certificate of Competence as an RPN, ability to travel to Fergus/Elora/Drayton/Arthur, and availability for full-time days with every other weekend and on-call shifts.

Qualifications

• Must hold current Certificate of Competence with the College of Nurses of Ontario as RPN.
• Willingness to attain SE Health certifications.
• Availability to work FT days with every other weekend and on-call component.
• Current CPR certificate is required.
• Must be willing to travel to Fergus/Elora/Drayton/Arthur and surrounding areas.

Responsibilities

• Complete comprehensive client assessments and develop care plans.
• Act as primary provider for a client caseload when designated, coordinating with the care team.
• Support clients through transitions by coordinating with community services.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements

• Current Certificate of Competence with the College of Nurses of Ontario as RPN.
• Availability to work FT days with every other weekend and on call component (Evening and Overnight 6 nights/month.)
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required SE Health certifications
• A current CPR certificate is required.
• This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
• Required to travel to Fergus/Elora/Drayton/Arthur and surrounding areas.
