Job summary

SE Health is seeking compassionate registered nurses to join our team in Amherstburg and surrounding areas. The role focuses on comprehensive client assessments, care planning, and evidence-based care delivery in collaboration with a multidisciplinary team.

You will serve as the primary provider for a client caseload, coordinating care with other team members and community services, while traveling to client locations as needed.

Qualifications

  • Current Certificate of Competence with the College of Nurses of Ontario.
  • Availability to work weekdays and minimum of 16 hours on weekends (Saturday and Sunday).
  • Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
  • A willingness to attain required SE Health certifications.
  • A current CPR certificate is required.
  • This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
  • Required to travel to Amherstburg and surrounding areas.

Responsibilities

  • Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs.
  • When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
  • Support the client through care transitions by collaborating and connecting the client with community support services in their local community.
