Job summary

SE Health in Ontario is hiring registered nurses who assess clients, develop personalized care plans, and implement evidence-informed care. The role supports transitions and works with a multidisciplinary team to deliver high-quality nursing care.

This full-time opportunity requires a College of Nurses of Ontario Certificate of Competence, shift flexibility (days/evenings/nights/weekends), immunization records, background checks, and CPR certification, with a willingness to attain SE Health

Qualifications

• Current Certificate of Competence with the College of Nurses of Ontario.
• Availability to work days, evenings, nights and every other weekend for 12 hour shifts.
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A current CPR certificate is required.
• Willingness to attain required SE Health certifications.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs.
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.
