Job summary

SE Health is seeking a Registered Nurse to provide high-quality in-home and facility care across Ontario, including Fergus, Elora, Drayton and Arthur. You will assess, plan and evaluate care for clients while coordinating with a multidisciplinary team.

The role offers a competitive compensation range ($37.35–$41.35/hour) with paid training, benefits, and flexible schedules. A sign-on bonus of $1500 and career growth opportunities are available.

Qualifications

• Current Certificate of Competence with the College of Nurses of Ontario as RN.
• Availability to work FT days with every other weekend and on call component (Evening and Overnight 6 nights/month).
• Willingness to attain SE Health certifications and a current CPR certificate.

Responsibilities

• Complete comprehensive client assessments, develop care plans, and evaluate care.
• Act as primary provider for a client caseload and coordinate with the care team.
• Assist clients through care transitions by connecting with community support services.

Responsibilities:

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements:

• Current Certificate of Competence with the College of Nurses of Ontario as RN.
• Availability to work FT days with every other weekend and on call component (Evening and Overnight 6 nights/month.)
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required SE Health certifications
• A current CPR certificate is required.
• This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
• Required to travel to Fergus/Elora/Drayton/Arthur and surrounding areas.
