Job summary

VON Canada is seeking a Visiting Nurse to deliver holistic, person-centred care in clients' homes, clinics, virtually, and in the community. You’ll work up to 0.5 FTE with a schedule spanning days, evenings, and weekends, including on-call duties, covering London and surrounding areas.

You will perform client assessments, wound care, chronic disease management, IV therapy, phlebotomy, dialysis management, rehabilitation, and end-of-life care, collaborating with other health professionals.

Qualifications

  • Must be licensed as a Registered Nurse in the province of practice.
  • CAPCE or equivalent required or agreement to complete upon hire.
  • Three years in community or acute care with focus on palliative care preferred.
  • Must own a vehicle, have a valid driver's license and proof of insurance.

Responsibilities

  • Client assessments to develop individual care plans.
  • Provide primary care in collaboration with other healthcare professionals.
  • Wound care, including simple and complex and negative pressure wounds.
  • Chronic disease management and diabetes education.
  • Medication management and IV therapy
  • Rehabilitative programs and palliative/end-of-life care.

Our Visiting nurses provide client care in the client’s own home. They provide physical assessments, symptom control support, palliative care, rehabilitation therapy, wound care, education, counselling to individuals and families and much more.

What you'll do:
  • Client assessments to develop individual care plans
  • Provide primary care in collaboration with other healthcare professionals
  • Simple and advanced wound care, including complex and negative pressure wounds
  • Chronic Disease Management, e.g. diabetes care and education
  • IV therapy and CVAD management
  • Phlebotomy and specimen collection
  • Dialysis Management
  • Rehabilitative programs
  • Palliative and End of Life Care
  • Medication management
  • And many other critical nursing skills
What we're looking for:
  • Must be licensed as a Registered Nurse in the province of practice
  • Community health experience is an asset
  • Fundamentals of Palliative Care, or an equivalent required
  • CAPCE or equivalent required or agreement to complete upon hire
  • Three (3) years of experience in the community or acute care setting, with a focus on palliative care is preferred
  • Must enjoy driving, have a personal vehicle and possess both a current driver's license and proof of vehicle insurance
  • A satisfactory Criminal Records Check is required; a Vulnerable Sector Search and/or Child Abuse Registry Check may also be necessary
  • Must be able to wear Personal Protective Equipment (PPE)
