Job summary

ParaMed Home Health Care in Ontario is seeking a Registered Nurse for community/home care. Experience in palliative care is mandatory, with a focus on autonomy and building long-term therapeutic relationships with patients.

You'll work with a supportive team, participate in hands-on skills labs and a comprehensive clinical orientation, and enjoy a full-time schedule with every other weekend off. Travel within the designated area is required.

Qualifications

• Registered Nurse with CNO in good standing.
• Valid driver’s license and access to a vehicle.
• Experience in delivering community care is an asset.
• Willingness to travel within designated area.
• On-call coverage after hours to ensure patient safety.

• Using appropriate assessment tools/techniques to assess patients’ individualized needs
• Executing and continuously monitor care plans
• Engaging patients using a collaborative approach by building rapport and trust
• Maintaining and update professional patient records related to patient encounters and plans
