Job summary

ParaMed Home Health Care in Ontario is seeking a Registered Nurse (RN) for community nursing visits, providing care to patients in various settings to help them remain at home as long as possible.

You will assess, implement, and monitor care plans, maintain patient records, and travel within the designated service area. The role requires on-call coverage and a willingness to travel between patient residences; bilingual English/French is preferred.

Qualifications

• Registration with the CNO as a Registered Nurse in good standing.
• Valid driver's license and access to a vehicle is required.
• Willingness to travel to patient residences within a designated area.
• Bilingualism (English/French) is preferred to support client care.

Responsibilities

• Using appropriate assessment tools/techniques to assess patients' individualized needs.
• Executing and continuously monitor care plans.
• Engaging patients using a collaborative approach by building rapport and trust.
• Maintaining and updating professional patient records related to patient encounters and plans.
