ParaMed Home Health Care in Belleville, Ontario is seeking a Part-time Registered Practical Nurse to provide compassionate, patient-centered care in community settings, within your scope of practice. Qualifications

• Registration with the CNO as a RegisteredPracticalNurse in good standing
• Valid driver’s license and access to a vehicle isrequired
• Experience in delivering community care is an asset
• Reliability and flexibility
• Willingness to travel to and frompatientresidences within a designated area
• Experience working with the vulnerable population
• Passion for making a difference in your community
• Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
• Bilingualism (English/French) is preferred to support client care

Responsibilities

• Participate in scheduled visits in the community to look after patient needs within your scope of practice
• Provide ethical and safe nursing care
• Useappropriate assessmenttools/techniques to assess patients’ individual needs
• Execute and continuouslymonitorcare plans and adjust asrequired
• Engage patients using a collaborative approach by building rapport and trust
• Maintain and update professional patient records related to patient encounters and plans
