AgeCare is seeking an experienced MDS Coordinator to oversee the MDS process for the interdisciplinary team in a long-term care setting. You will supervise MDS documentation, provide training, and ensure timely assessments across the residence.

The RAI (Resident Assessment Instrument) Coordinator is responsible for the overall administration, organization, education, coordination, and implementation of the Minimum Data Set (MDS) for the interdisciplinary team. Provides supervision and assistance to the interdisciplinary team regarding MDS documentation.
Salary Range: $69,000.00 - $71,000.00 per year Salary placement based on qualifications and experience.
