Southbridge Roseview in Thunder Bay, Ontario is seeking an experienced RAI Clinical Coordinator to support residents through MDS assessments, documentation, and care-team coordination.

The role focuses on timely MDS completion, integration with PCC software, staff training, data analysis, and ensuring accuracy of RUG classifications; you will liaise with Health Zone, CIHI, AH&W and PCC.

Qualifications

  • 3–5 years' Long Term Care experience; MDS experience is an asset.
  • Registered as a RPN under CNO.
  • Knowledge of nursing rehabilitation and OT/PT.
  • Strong analytical and assessment skills.
  • Ability to lead training for adult learners.
  • Knowledge of InterRAI tools and MDS/RUG III classifications.

Responsibilities

  • Coordinate MDS assessments within established timelines.
  • Integrate MDS with clinical tools and streamline processes.
  • Provide data analysis across Resident and Home levels.
  • Explain MDS strengths to staff.
  • Identify financial implications of coding inconsistencies.
  • Liaise with Health Zone/AH&W/CIHI/PCC.
  • Develop PCC program materials as required.
  • Respond to Health Zone submission errors and corrections.
  • Coach facility staff to understand RUG classifications.
  • Develop processes to improve RAI content accuracy.
  • Support audits of RAI assessments for accuracy.
  • Participate in strategic program development.

Reports to: Director of Care

The RAI Clinical Coordinator supports residents by managing assessments, ensuring accurate documentation, and helping the care team provide the best possible environment and care.

This role will include duties such as:
  • Coordinatingthe completion of MDS assessment on all residents within established timelines
  • Understandinghow the MDS assessment integrates with existing clinical tools and provides leadership in streamlining these processes
  • Understandingand providingcritical analysis of data at different levels of aggregation (Resident, Home Health Zone, etc.)
  • Articulatingfor staff the strengths of MDS as a clinical assessment tool
  • Articulatingthe financial implications of coding inconsistencies
  • Acting as key contact and liaison with: Health Zone/AH&W/CIHI/PCC, etc.
  • Developingfacility specific material within the PCC program as required
  • Responding to Health Zone on all submission errors
  • Facilitatingresponse and correction of submission errors with facility staff
  • Coaching tokey facility personnel to build capacity for understanding RUG patient classification levels
  • Developingand introducing processes, and systems to improve accuracy of RAI content
  • Facilitateaudits of RAI assessments for content accuracy
  • Participates in strategic planning for program development/improvement
  • Responsible for Clinical and Admin Setup role to support MDS within PCC software program
  • Interprets reports generated by Health Zone/AH&W or CIHI and provides critical analysis for Home
  • Understands the development of RAP’s, quality indicators, and RUG case mix products
The qualified candidate will have:
  • 3-5 years' experience in Long Term Care, experience with MDS an asset
  • Registered as a RPN under CNO
  • Knowledge in nursing rehabilitation and working OT and PT
  • Strong analytical and assessment skills
  • Ability to lead training and education with Adult learners
  • Knowledge of: InterRAI suite of tools, MDS/RUG III patient classification systems.
Additional Requirements include
  • A negative vulnerable sector check completed within 6 months of start date.
  • Completion of a 2 Step TB Skin test within the last 12 months of start date.
