Qualifications

• Strong analytical, critical thinking, and problem-solving skills.
• Proficient in Word and Excel; able to manage data and generate reports.
• Experience with electronic health record applications, preferably PointClickCare and Point of Care.

Responsibilities

POSITION SUMMARY

Reporting to the Executive Director, the RAI Coordinator supports the coordination, integrity, and oversight of the interRAI LTCF assessment process by ensuring assessment are completed, validated, and submitted in accordance with legislative requirements, CIHI standards, and organizational policy.

The RAI Coordinator acts as the home’s subject matter expert for the interRAI LTCF, supporting accurate, timely, and resident-centred assessment practices that inform care planning, quality improvement, and case-mix classification.

CORE RESPONSIBILITIES

Assessment Coordination & Submission

• Coordinates and monitors completion of all interRAI LTCF assessments within required timelines
• Supports interdisciplinary team participation and collaboration to reflect resident status, preferences, and goals
• Validates assessments prior to locking to ensure accuracy, completeness, and internal consistency
• Confirms Collaborative Action Plans (CAPs) are reviewed and inform care planning prior to finalization
• Submits assessments to CIHI and addresses errors, warnings, and rejections, as applicable
• Conducts routine audits to ensure alignment between clinical documentation, Point of Care (POC) charting, and interRAI coding, with a focus on key areas (e.g., ADLs, mood/behaviours, and clinical treatments), ensuring all assessments are supported by clear, accurate, and defensible clinical documentation.
• Identifies patterns of coding discrepancies and sources of variability across assessors and disciplines, providing feedback to support consistency, data integrity, and reliable case-mix and quality reporting

Clinical Integration & Care Planning

• Supports integration of interRAI LTCF outputs into clinical practice, including care planning, risk identification, and clinical decision-making
• Coordinates interdisciplinary care plan reviews, to support alignment of assessment findings with resident-centred care

Data Analysis & Quality Improvement

• Interprets and analyzes Quality Indicators (QIs), outcome measures, and key clinical data at the resident and home level
• Monitors Case-Mix Index (CMI) and Resource Utilization Groups (RUG-III Plus) and supports understanding of data implications
• Shares insights with leadership and interdisciplinary team members to support quality improvement initiatives
• Participates in organizational quality improvement, risk, and strategic planning activities

Education & Capacity Building

• Acts as a resource and provide education and coaching to interdisciplinary team members on interRAI LTCF coding standards, assessment processes, and use of outputs
• Supports onboarding and ongoing competency development of assessors, including completion of required training and competency evaluations
• Coaches personal care staff on Point of Care (POC) documentation practices to support accurate assessment coding
• Promotes ethical, accurate, and resident centered assessment practices

Systems, Reporting & Liaison

• Serves as the home’s point of contact for interRAI-related processes and data inquiries

• Interprets Canadian Institute for Health Information (CIHI) and internal reports, communicating findings to leadership in a meaningful and actionable way
• Maintains current knowledge of interRAI LTCF standards, CIHI reporting requirements, and applicable legislation

QUALIFICATIONS

• Current registration as a Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario
• Minimum 3-5 years’ experience in long-term care, interRAI experience preferred
• Completion of required interRAI LTCF training and competency evaluation
• Demonstrated knowledge of case-mix systems, including RUG-III Plus methodology
• Strong analytical, critical thinking, and problem-solving skills
• Proficient in computer applications, including Microsoft Word and Excel, with the ability to manage data, generate reports, and support analysis related to interRAI LTCF assessments
• Proficiency with electronic health record applications, preferably PointClickCare and Point of Care
• Demonstrated ability to lead education sessions and support adult learning
• Knowledge of quality improvement methodologies and data-driven decision-making
• Effective communication and collaboration skills
