Job summary

Home Care Assistance is seeking a PSW to support residents in an LTC facility in Collingwood and Penetanguishene. Shifts may vary from mornings to evenings to overnights, with every other weekend. Two plus years of experience is required for this role.

Qualifications

• PSW certification is required.
• Valid First Aid/CPR certificate.
• Police Vulnerable Sector Check within last 6 months.
• CPI training is an asset.
• Driver's license and own insured vehicle mandatory.
• Satisfactory professional references.

Responsibilities

• Provide personal care.
• Maintain open communication with client and family.
• Monitor care plan and document changes.
• Respond to emergencies.
• Follow Care Plan and ADLs.
• Provide cognitive, social, emotional, and spiritual support.
• Maintain clear and detailed care notes.
• All other duties as assigned.

The successful candidate’s areas of responsibility will include, but are not limited to, the following:

• Provide personal care
• Drivers license and reliable car are mandatory for this position.
• Providingcognitive, social, emotional, and spiritual support
• Provide behaviorsupport and crisis intervention
• Maintaining open communication with client and their family
• Monitoring care plan and adding documentation with any changes and updates
• Responding to all emergency situations
• All other duties, as assigned

Key qualifications for this position will include, but are not limited to:

• PSW certification*required
