Job summary

Qualicare North York Homecare is seeking an experienced Personal Support Worker for a client at Baycrest Rehab Centre (Bathurst & Wilson) in Toronto. Shifts are Thursdays & Fridays from 8am to 2pm, with opportunities to pick up additional shifts for other clients.

This role requires PSW certification, CPR/First Aid, and ability to lift/move clients. Strong teamwork and reliable communication with family are essential to provide compassionate care.

Qualifications

• PSW certificate is required.
• CPR/First Aid certification required.
• Vulnerable Sector Screen or willingness to obtain one.
• Ability to lift/move clients.
• Negative TB skin test or chest x-ray.
• Access to internet or mobile phone.
• Excellent teamwork and support.

Responsibilities

• AM care and client assistance.
• Escort client to dining room and assist with feeding.
• Light housekeeping and transportation assistance; walker/wheelchair use.
• Provide medication reminders and accompany on outings.
• Provide companionship and follow care plan.
• Documentation and charting; communicate with family.

Job Summary

The Personal Support Worker is a member of the home care team who works under the supervision of the Director of Care and performs various personal care services as necessary to meet the client’s needs. Work is one on one.

Qualicare North York Homecare is looking for an experienced PSW to join our team for a client at Baycrest Rehab Centre (Bathurst & Wilson). Shifts: Thursdays & Fridays: 8am-2pm. You will have the opportunity to pick up other clients on other days as well.

Responsibilities

• AM care
• Escort client to dining room and assist with feeding
• Light Housekeeping
• Accompany clients on outings; client uses walker and wheelchair
• Medication Reminders
• Ambulation and transporting of client
• Provide companionship to client
• Communicate with client's family that visits regularly, accompany on outings
• Ability to follow a care plan
• Documentation/Charting

Qualifications

• Personal Support Worker Certificate.
• Current CPR/First Aid training.

• Ability to lift/move client as client mobility is declining.
