Job summary

ParaMed Home Health Care is seeking a pediatric-focused community RN/RPN to provide in-home visits, assess patients, and manage individualized care plans. You will work with families to ensure comfort and safety in the home setting while coordinating care with a supportive team.

The role requires CNO registration as an RN with the ability to travel within designated areas; full-time hours; comprehensive benefits and ongoing training are provided.

Qualifications

  • Must be registered with the CNO as a Registered Nurse in good standing.
  • Valid driver’s license and access to a vehicle are required.
  • Experience delivering community/home care is an asset.

Responsibilities

  • Assess patients using appropriate tools to identify individualized needs.
  • Execute and monitor care plans.
  • Engage patients with a collaborative approach to build rapport and trust.
  • Maintain and update patient records related to encounters and plans.

In this community Pediatric RN Shift Nursing role, as a Registered Practical Nurse (RPN) you will have scheduled visits to care for patients in various settings. You will work with patients of all ages with differing complexity of palliative care needs and play a key role in helping patients remain comfortable in their homes for as long as possible. Experience with children is mandatory for this position.
