Job summary

ParaMed is seeking a Registered Nurse (RN) or Registered Practical Nurse (RPN) for community nursing visits across designated areas. You will assess patients’ needs and implement care plans while building trusting relationships to support comfortable, in-home care.

You must have CNO registration in good standing and a valid driver’s license with willingness to travel. On-call coverage after hours is required, and you’ll join a team offering training, benefits, and growth opportunities.

Qualifications

• Registration with the CNO as a Registered Nurse or RPN in good standing.
• Valid driver’s license and access to a vehicle are required.
• Experience in delivering community care is an asset.
• Reliability and flexibility to travel within a designated area.
• Willingness to travel to patient residences and provide on-call coverage after hours.

• Using appropriate assessment tools/techniques to assess patients’ individualized needs.
• Executing and continuously monitor care plans.
• Engaging patients using a collaborative approach by building rapport and trust.
• Maintaining and update professional patient records related to patient encounters and plans.
