Job summary

ParaMed Home Health Care in Toronto is seeking a Registered Practical Nurse (RPN) for community nursing with scheduled visits to care for patients in their homes. You will work with patients of all ages, assess needs, and help keep them comfortable at home for as long as possible.

Qualifications

• Registration with the CNO as RN/RPN in good standing.
• Valid driver’s license and access to a vehicle.
• Experience in delivering community care is an asset.
• Reliability and flexibility.
• Willingness to travel to patient residences within a designated area.
• On-call coverage, including after hours to ensure patient safety.

Responsibilities

• Using appropriate assessment tools/techniques to assess patients’ individualized needs
• Executing and continuously monitor care plans
• Engaging patients using a collaborative approach by building rapport and trust
• Maintaining and update professional patient records related to patient encounters and plans
