Job summary

ParaMed Home Health Care is seeking a Registered Practical Nurse (RPN) to provide community-based nursing to patients in their homes across Canada. You will perform scheduled visits, deliver ethical and safe care, and collaborate with RNs for complex cases, supporting autonomy and home-based care.

The role offers part-time and full-time options, flexible scheduling, and opportunities to develop clinical skills within a supportive team.

Qualifications

Registered with the CNO as a Registered Practical Nurse in good standing.
Valid driver’s license and access to a vehicle.
Experience in delivering community care is an asset.
Reliability and flexibility.
Willingness to travel to patient residences within a designated area.
Experience working with the vulnerable population.
Passion for making a difference in your community.
Required on-call coverage, including after hours to ensure patient safety and care needs.

Responsibilities

Participate in scheduled visits in the community to look after patient needs within your scope of practice.
Provide ethical and safe nursing care.
Use appropriate assessment tools/techniques to assess patients’ individual needs.
Execute and continuously monitor care plans and adjust as required.
Engage patients using a collaborative approach by building rapport and trust.
Maintain and update professional patient records related to patient encounters and plans.
