Job summary

The Salvation Army – Crossroads Residential Services in Canada is seeking a Caseworker (Outreach). You will render direct casework and group work to assist clients, while supporting the partnership with the property management company.

You will be part of the Case Management Team for Crossroads Residential Services. This role involves maintaining thorough records, facilitating wellness checks, and coordinating with external social service agencies.

Qualifications

• Two-year post-secondary program in social services or related field.
• First Aid/CPR certification.
• Criminal Record Check including Vulnerable Sector.
• Class 5 driving license with driving record.

Responsibilities

• Provide direct casework and group work to assist clients.
• Maintain client files and documentation (SAMIS).
• Coordinate with property management and community partners.
• Attend case conferences and support discharge planning.
