Job summary

The Salvation Army Canada and Bermuda Territory is seeking a social services professional to perform focused case management, client assessments, goal setting, and discharge planning. You will coordinate with health, law enforcement, and community partners to support client outcomes while ensuring compliance with program requirements.

Qualifications

• Completion of College diploma in Social Services/Work, Criminology, Psychology, Correctional Services, or related fields.
• Certification in First Aid and CPR.

Responsibilities

• Provide comprehensive case management services, including client assessments, case planning, coordination, documentation, and referrals.
• Collaborate with multidisciplinary partners such as law enforcement, healthcare providers, Indigenous organizations, and social service agencies.
• Contribute to program development by offering feedback on program design and policies.
• Assist with program delivery and administration, including data tracking and monthly reporting.
• Facilitate referrals and applications for clients to access community resources or services.
