Job summary

The Salvation Army in Canada is seeking a dedicated case manager to support clients with social adjustment challenges, discharge planning, and goal setting. You will work across program sites, maintain records, and collaborate with health, law enforcement, Indigenous groups, and social services to ensure strong outcomes.

Strong communication, empathy, and organizational skills are essential to guide clients through complex processes, with a focus on safety, dignity, and respectful service

Qualifications

• Completion of College diploma in Social Services/Work, Criminology, Psychology, Correctional Services, or related field.
• Certification in First Aid and CPR.
• At least 2 years of related experience is preferred.

Responsibilities

• Provide comprehensive case management, including assessments, planning, coordination, documentation, and referrals.
• Collaborate with multidisciplinary partners to support client outcomes.
• Contribute to program development and policy implementation across sites.
• Assist with data tracking, reporting, and project summaries.
• Facilitate referrals to community resources and follow up on outcomes.
• Conduct home visits and deliver in-home support aligned with program requirements.
• Engage stakeholders through outreach and partnership development.
• Uphold OH&S standards and organizational policies in daily duties.
