Job summary

The University Health Network (UHN) at Toronto General Hospital is seeking a temporary full-time Social Worker to support the Transplant program for 12 months. The role involves ambulatory care and collaboration with an inter-professional team.

You will assess patients, develop and implement intervention plans, liaise with community agencies, and contribute to teaching and research. Requirements include a MSW, provincial registration, and CKD experience.

Qualifications

• Masters in Social Work required
• Registration with the Ontario College of Social Workers and Social Service Workers required
• Experience in ambulatory care is required
• Experience working with chronic kidney disease populations is required
• Excellent verbal and written communications skills

Responsibilities

• Assess patients to evaluate need for service and formulate recommendations
• Implement intervention plans with patients and families
• Reassess and modify plans as needed
• Liaise with community agencies
• Complete applications and exchange information with inter-professional team
• Provide clinical teaching to students and colleagues
• Participate in research activities
• Maintain efficient operation of the Social Work group
